Award Criteria

  • Demonstrated impact of (formal or informal) leadership roles/positions
  • Long term contributions to CPD
  • Demonstrated impact of innovation(s)
  • Demonstrated sustained commitment to and excellence in education (which may include teaching, program coordination, administration, and/or scholarly activities in CPD)

Eligibility

  • All faculty (affiliated with University of Toronto Faculty of Medicine) need to have a minimum of 10 years of academic and/or clinical experience for consideration
  • Please refer to Terms of Reference for further details

Nomination Requirements

When completing the online form you will be asked for the following:

  • Short statements on how the nominee meets each criterion (around 300 words per statement)
  • Confirmation that the department chair/vice chair of education supports this nomination, on the basis that the nominee’s accomplishments align with the award criteria
  • A short peer testimony (up to 300 words)
  • Up to date curriculum vitae highlighting CPD roles (eg grants, publications, invited lectureships, mentorships, directorships, etc.)

Optional Submission Documents

  • Material or data that supports the nomination and highlights the commitment, impact and successes of the nominee. Evaluation data can be included, but please do not send raw data. Summaries or aggregated data are preferred
